Walk into a typical care group's finance review and you'll find one line item bigger than every other operational variance combined: agency. It's not new, and it's not surprising — but the size of it (in 2025-26 our reference set is averaging 17.4% of payroll, peaking at 31% during winter pressures) keeps surprising operators because nobody tracks it weekly.

1 · Open-shift fill time
When a shift opens, how long does it take to fill from your own staff? Operators who track this hit a 14-minute median; operators who don't are at 4+ hours and reach for the agency phone before they've even tried.
2 · Cert expiry recovery rate
Of the staff whose certs expired in the last 90 days, how many were renewed within 14 days? Below 70% means you can't legally schedule them — and that's where agency creeps back in. It's a compliance KPI dressed up as a finance one.
3 · Bradford Factor watchlist size
The number of staff sitting at Bradford 200+. These are your future absences. Sit with the watchlist for 20 minutes a week with your HR business partner; agency cost trends down within a quarter.
